Getting Started
Every client, whether it’s an individual or entity will receive a free 15 minute virtual screen.
The purpose of this screen is to get an idea of the clients’ needs, determine if Hemlock Health and Wellness is an appropriate source to meet the need, and identify next steps.

Payment for services:
No insurance accepted
You have the right to receive a good faith estimate for your services before they are rendered
Documentation can be provided for insurance company submission upon request
Credit and debit can be accepted through Simple Practice
Venmo and Apply Pay are also accepted
Cash or personal checks are not accepted.
OT Plan of Care Process
Evaluation completion
■ Assessment of the areas of concern/reason for referral
■ Acquisition of subjective and objective data
Plan of care
■ Established goals (both short and long term), frequency, duration, intervention codes
Update of the plan of care
■ Any change in the plan of care including treatment intervention codes, goals, restrictions, precautions, or major shifts in the medical or psychosocial dynamics
Treatment encounter note
■ One for each intervention session with coding
Addendum note
■ Non-billable, information pertaining to the case but may not be involved in the intervention time
Discharge summary
■ Summary of goals, progress, barriers, facilitators and recommendations

OT Evaluation Process
Starts with a virtual screen
Three tiers of evaluations based on the complexity of the clients medical conditions and needs. The tiers of evaluations have different prices. There is a flat fee for the evaluation and the development of a plan of care depending on the complexity of the client to be determined by the evaluating therapist.

Consultation, Evaluations & Treatment
Treatment is then billed based on time. Clients purchase time based on the recommendations coming from the evaluation (frequency and duration of treatment). The client is in control of the time they purchase and use with the understanding that the
ability to reach goals that are established depends on the frequency and duration of time recommended. Each treatment code billed for time rendered is associated with a different fee/cost.
Depending on the location of the client, the initial
evaluation can be virtual as appropriate, or in person if the client is within a drivable distance.
For OT consultation, there is a flat fee for the initial consultation meeting (60-90 minutes) which includes the development of a needs assessment (developed after the initial meeting) highlighting the identified need and plan to remediate or meet the need. This can be virtual or in person depending on the location of the client
Further consultation fees will be individually determined based on the scope of the project. A retainer may be required.
Should clients not be within a drivable distance, there are associated costs to the client for travel on a case by case or project by project basis.
